Beginning with the 2005-2006 season IHOA adopted the AHAI requirements that every person over the age of 18 must be screened for any conviction of criminal activity. You only need be screened once in your career. If your name is on the AHAI website as having been screened as an AHAI coach or manager you do not have to be screened again. If you were screened at our Initial Screening Drive at your IHOA Seminar last year, you do not need to be screened again. To check to see if your name appears on the AHAI website as having already been screened click on the link below:

You must use the AHAI approved Screening Service. If you have not been screened and need to accomplish this please follow these simple directions. Please print out the IHOA Screening Packet Request Form, complete it and mail it in with the applicable fee to the address listed below.

To obtain a Screening Packet with instructions you must complete the form above and send a check made payable to AHAI in the amount of $58.00 to:

Please include the full name of the individual(s) you wish to be screened and a return address as to where to mail the screening forms. These forms will be mailed within 24 hours of receipt of your request and full payment.
